Telehealth Consent

This page summarizes what you will be asked to agree to when you start an online visit. The full consent is presented on the clinical platform (operated by HOO Longevity, our clinical platform partner) before any visit, and that version controls.

What telehealth is

Telehealth means a licensed provider evaluates you using information you submit online, and in some cases by phone or video, instead of an in-person appointment. The provider may prescribe, decline to prescribe, or recommend in-person care.

What you agree to

  • You are 18 or older and the information you provide is accurate and complete.
  • You understand that telehealth has limits. A provider cannot examine you physically and may decide that in-person care is more appropriate.
  • You understand that the provider decides whether any treatment is appropriate, and that no result is promised.
  • You understand that some medications are compounded for an individual patient and that compounded medications are not FDA-approved.
  • You consent to communication through the secure patient portal and by the contact details you provide.
  • You understand that NewDay Wellness provides no medical services.

Your rights

You may withdraw consent and stop at any time. Doing so may end the program. Your information is protected under HIPAA on the clinical platform.

Emergencies

Telehealth is not for emergencies. Call 911.
